Nomination Form for the Judith Chapman Memorial Women's Leadership Award



NOMINATE AN OUTSTANDING JEWISH FEMALE VOLUNTEER AND LEADER!

The Judith Chapman Memorial Women's Leadership Award honors those who serve as role models in our community.

Recipients of this high honor:

  • Demonstrate significant and sustained volunteer leadership in the Jewish communities of San Francisco, the Peninsula, Marin and Sonoma Counties, and the East Bay;
  • Place a high value on their Jewish identity;
  • Model the importance of tzedakah in their lives;
  • Have a significant connection with the Federation;
  • Demonstrate volunteer leadership in the secular community;
  • Mentor, motivate, and encourage others to follow in their footsteps.

A $2,500 grant will be awarded and designated to a nonprofit of the recipient’s choice.

Nomination Instructions

You may seek feedback from others, including professionals who have worked with the nominee. You may re-nominate previous nominees who have not received the award. Please include as much information as possible for the Selection Committee to consider.

  1. Prepare your answers and save the document periodically as you work.
  2. Copy and paste your answers into the appropriate fields on the nomination form below.
